Is my insurance plan accepted?
We accept most insurance plans and cash patients. Please contact us to inquire about your specific insurance plan.
How much will my copayment be?
Benefits will be verified once we receive your insurance information. Please have your insurance ID card ready when scheduling your first visit. If there any changes to your insurance plan, please call and notify us.
What is your new patient procedure?
Please bring your ID, insurance card and medications to your new patient visit. If you have any imaging or labs that you would like to be added to your chart, please bring those as well. Please arrive 15-30 minutes before your appointment time to fill out any paperwork.
Are Telehealth visits available?
Yes, we offer Telehealth visits for your convenience. Patients may be asked to visit the clinic for lab or blood work as needed. We are also available in person at our Carrollton primary care clinic.
